1 Position Overview
Quality of Life Group is seeking a compassionate and dedicated Care Manager to join our Care Management team. In this role, you will provide comprehensive assessment, planning, and coordination of care for older adults and their families, helping them navigate the complex healthcare system with dignity and expertise.
You will work directly with clients to develop personalized care plans, coordinate services, and serve as an advocate for their wellbeing. This is an opportunity to make a tangible difference in the lives of seniors and their caregivers while growing your career in gerontology.
2 Essential Duties & Responsibilities
- Comprehensive Assessments: Conduct biopsychosocial assessments to evaluate client needs, functional abilities, and care requirements
- Care Planning: Develop and implement individualized care plans based on assessment findings and client goals
- Care Coordination: Coordinate with healthcare providers, facilities, and community resources to ensure continuity of care
- Family Support: Provide education and support to family caregivers and involved parties
- Documentation: Maintain accurate and timely clinical records in accordance with agency standards and regulatory requirements
- Community Outreach: Build and maintain relationships with community partners to expand resources for clients
3 Minimum Qualifications
Education & Experience
- Bachelor's or Master's degree in Social Work, Nursing, Gerontology, or related field
- 2+ years of experience in case management, healthcare, or social services
Required Skills
- Strong assessment and care planning skills
- Excellent communication and interpersonal abilities
- Proficiency with EMR/EHR systems and Microsoft Office
- Valid driver's license and reliable transportation
